- A claim application and a self-attested photocopy of both sides of the ticket are required.
- Two passport-sized photographs of the lottery winner, attested by a Gazetted Officer/Notary.
- A receipt for the award money in the specified form, with a 1/- revenue stamp (Download Receipt Here).
- A self-attested copy of the winner’s PAN Card.
- ID Proof Documents such as Adhaar Card, Ration Card, DL, Passport, Voter ID Card, and so on.
